Claims Procedure
Dear Administrator:
In providing proper risk management for your school you should be aware of the procedures involved in filing a claim when a student is injured. Please read below to understand the process for filing a claim.
Process for Filing a Claim:
- Have the appropriate administrator complete part one of the claim form. Claims forms are included in your administrator's kit.
- Have the parent complete part two of the claim form and mail it to the address located at the top left corner of the claim form along with any itemized bills, being certain that the bills include treatment dates and the diagnosis.
- Student Accident coverage pays after any other insurance therefore along with the claim form and itemized bills, copies of all Explanation of Benefits received from other insurance carriers will need to be included.
- Please make sure that all medical providers are aware that the Student Accident benefits is secondary insurance coverage and will only pay after the primary carrier has paid or denied a claim.
- To check on the status of a claim call 1-866-653-2542. Be sure to have the Policy Number, Student's Name and Date of Injury.
